Day Treatment Care CoordinatorThe Day Treatment Care Coordinator provides psychoeducation, skill building and care coordination to children and families with serious emotional and behavioral challenges. Services may be provided in the school, home or community setting, depending on the client's needs.QualificationsMaster’s or bachelor’s degree in a human services field from a college or university included in the US. Department of Education’s database of accredited schools, ORQualified Addiction Professional (QAP), ORAssociate of Applied Science degree in Behavioral Health Support as designated by the Department of Mental Health.Bachelor's degree unrelated to human services field with two years of qualifying experience; ORFour years of qualifying experience.Qualifying Experience must include delivering services to individuals with mental illness, substance use disorders or developmental disabilities. Experience must include at least one of the following:Providing one-on-one or group services with a rehabilitation/habilitation and recovery/resiliency focus.Teaching and modeling for individuals how to cope and manage psychiatric, developmental, or substance use issues while encouraging the use of natural resources/supports.Supporting individuals in their efforts to find and maintain housing, employment, and/or function appropriately in a family, school, and community.Assisting individuals to achieve the goals and objectives on their individualized treatment plan.Core Job DutiesProvide psychoeducation to clients with serious emotional and behavioral challenges to help them develop coping strategies, so they can remain in the school, community and in the home.Implement treatment goals according to person-centered care plan.Develop and review critical intervention/safety plans to provide immediate support during emotional or behavioral crises.Facilitate communication between families, schools, and service agencies.Provide transportation in the staff's personal vehicle or agency vehicle (if available) when medically necessary and appropriate.Educate clients and families on interventions and behavior modification techniques to better manage behavioral health conditions.Link to community resources and help families/clients develop natural supports.Help clients to develop or improve interpersonal, social, family, community and daily living skills.Attend treatment team meetings and prepare clients for transition out of day treatment into the regular classroom.Complete required documentation in a timely manner.Maintain certification in CPI and CPR/First Aid.Adherence to all applicable evidence-based practice models.Orally communicate information effectively and accurately.Assure program operates within the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care.Abide by program and agency policies and procedures.Other job duties and special projects as assigned.
